I believe the images would need to be distorted. Otherwise an image hash could be compared really efficiently by a machine with access to Flickr. That is, once it has indexed Flickr and hashed all images with the targeted tags, it would simply hash the images presented in the CAPTCHA. After that, it is game over for this time.

Still, I believe a solution is getting nearer and that it does involve images.
